Clinical triage and initial discussion
A senior member of our clinical team reviews the referral and contacts the referrer or family to discuss needs, clinical requirements, and immediate priorities.
Comprehensive needs assessment
We carry out a thorough assessment of the individual's clinical needs, home environment, equipment requirements, and personal preferences.
Care plan, staffing, and training
A detailed care plan is developed and costed. We recruit, train, and brief the care team specifically for the package before care begins.
Care commences and is reviewed
Care begins at the agreed start date. We review the package regularly and report routinely to referrers, commissioners, and families.
